1. Simply fill out the form below and you will be given a confirmation number and an e-mail address on the confirmation page.
2. Submit a digital photo of your family pet in Mets' gear, along with the confirmation number to the e-mail address given.
There will be 4 prizes awarded after each of the remaining judging dates (May 22, June 6, June 27, July 14, July 28, August 11, August 25 and September 11 of 2007). All family pets are welcome!
